Question 1

Install fails or rolls back?

Accepted Answer

Disconnect the US-122L, run the installer as administrator, and temporarily disable antivirus. Remove any previous TASCAM US-122L driver entries in Device Manager, reboot, then reinstall.

Question 2

Device not detected?

Accepted Answer

Use a direct USB port instead of a hub, try a known-good USB cable, and confirm the interface powers on. Check Device Manager for an unknown USB audio device and reinstall the driver.

Question 3

Version conflicts?

Accepted Answer

Uninstall older TASCAM drivers and bundled audio utilities first, reboot, then install the correct US-122L package. Select the US-122L as the audio device in Windows and your DAW.
